The Final Forms registration/update window is open for the Loudonville-Perrysville 2021-22 school year. Parents are required to annually update their students' forms by verifying that their student information is correct or make changes and sign the form(s). The process should only take a few minutes as the data is saved from year-to-year. Parents are also reminded you may update your students' forms anytime throughout the school year with changes such as contact information, health history, medication, etc.
To update your student(s) be sure you are registering for the 2021-22 school year. You will be prompted to click the big blue button on the My Students page under your Parent account:

Please note: you must complete and/or update your student information BEFORE your student (grades 7-12 only) can sign their forms.
