Welcome to REDBIRD country! If you are moving into the District and would like to enroll your student(s) in the Loudonville-Perrysville Schools please follow these steps:
Log onto Final Forms, set up a parent and a student account and complete the required student forms. Click HERE to start the process.
Phone Tia Bowman in Central Enrollment to set up an enrollment appointment. 419-994-3912 option 4. Appointments are available Monday-Friday, 8:00-3:00, located at the C. E. Budd School, 210 East Main Street, Loudonville. Appointments are approximately 30 minutes.
a. Bring to the appointment: students birth certificate, immunization records, custody/guardianship documents (if applicable), proof of residency and your photo ID.
b. If your student has Gifted or Special Education needs please bring the most current documentation.

Kindergarten 2026-2027 Registration
Registration for 2026-27 kindergarten students in the Loudonville-Perrysville School District will begin Monday, February 23, 2026. Your child must be 5 years old on or before the district's first day of school for the 2026-27 school year (August 18th, 2026), to enter kindergarten. To help guide you through the process please read the following information.
The first and most important step is to complete the Final Forms registration. Please be sure that you are registering for the 2026-2027 school year. For instructions to complete Final Forms, click HERE.
After completing Final Forms, contact Tia Bowman, Enrollment Coordinator, at 419-994-3912, option 4 to schedule an appointment to complete the enrollment process. Be sure to bring the following documents to your appointment as required by state law.
Child's birth certificate
Child's immunization record
Your state ID or driver's license
Current proof of residency (rental agreement or utility bill)
A complete copy of adoption or custody documents, if applicable
Your child must also have a physical examination before entering kindergarten. To download an exam form, click HERE.
Oftentimes, parents wonder if their child is ready for kindergarten. Your child should be physically, socially, and emotionally ready to start the school year. Open Enrollment
Thank you for considering the Loudonville-Perrysville EVSD as the school to educate and care for your child! Applications must be submitted no later that June 1, 2025, for the 2025-2026 school year. A separate form is required for each student and a new application is required for each school year. Once the application has been approved or denied, you will receive a letter in the mail. Those who are approved will be directed to log into Final Forms to complete all beginning of the year and additional forms, as needed. Thank you!
